Job Description:

Summary: Applies specialized clinical knowledge and advanced clinical skills in the areas of assessment, treatment of medical/emotional/behavioral disorders, and discharge planning. Detailed responsibilities: • Interviews and assesses patients and/or family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. • Initiates case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with the patient's treatment plan. • Designs and implements patient and family support groups. • Provides crisis intervention for patients and family members. Supports the hospital team with Baker Acts and evaluations. • Assists with discharge planning and processes. May identify appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. • May supervise and/or train new staff, students, and interns.
