Job SummaryWe are seeking a skilled RN to join our Care Coordination Center team at Memorial Healthcare System. As a Care Coordination RN, you will play a critical role in ensuring the smooth transfer and placement of patients across our facilities.Key Responsibilities:

• Coordinate patient transfers and bed placements, ensuring timely and efficient movement of patients.
• Communicate effectively with surgery departments, emergency departments, nursing units, physician offices, and outlying facilities to ensure seamless transfer and placement.
• Monitor and document transfer needs, communicating patient condition to sending or receiving providers and facilities.
• Consult with other disciplines to ensure patient needs are met, including infection control, equipment needs, nutritional, functional, pain, social, and skin integrity.
• Serve as an expert in Emergency Medical Treatment and Labor Act (EMTALA) and intervene as needed, escalating to leadership if required.
• Follow established protocols for interfacility transfer acceptance and escalate to direct report, Administrative Officer, or Administrator On Call (AOC) as necessary.
• Document transfer and bed placement in the Epic EMR, ensuring adequate patient safety standards are met.
• Collaborate across disciplines to coordinate patient transfers, including disposition, ambulance transportation, specialist referrals, and more.Requirements:
  • Bachelor's degree in Nursing (Required)
  • Registered Nurse License (RN LICENSE) - State of Florida (FL)
