Job Description:
Summary:The Peritoneal Dialysis Nurse is responsible for
providing Peritoneal Dialysis care services to patients and
families in collaboration with a physician, nurse practitioner,
social worker, and other members of the interdisciplinary
palliative care team.Detailed responsibilities:* Supports patients
and their families maintaining a multidisciplinary approach to
coordinate established peritoneal dialysis protocols and establish
an appropriate plan of care.* Provides expert nursing care to the
pediatric peritoneal dialysis patients and their families,
including ongoing management of dialysis needs, patient assessment,
patient education, phone triage, data collection, clinic visits as
appropriate and on call requirements.* Performs initial, 90 day,
and annual patient assessment for the pediatric peritoneal dialysis
patient. Determines need for reassessment if patient is unstable
and communicate with the multidisciplinary team.* Develops and
maintains a plan of care for all patients and update as needed.*
Coordinates the scheduling of all pediatric peritoneal dialysis
patients home training.* Coordinates care of pre and active
dialysis.* Serves as a resource for all pediatric peritoneal
dialysis treatments within the program.* Communicates and
collaborates with the medical staff, dialysis nursing staff,
dialysis social worker, dietician regarding the management of the
pediatric peritoneal dialysis patient.* Coordinates and schedules
peritoneal dialysis training for all new ESRD patients and
families. Communicates the training plan to the family, nursing
staff and Interdisciplinary team.* Evaluates the patient s and
families learning needs and design and implement a peritoneal
dialysis training program to meet their needs.* Assess vitals,
height, weight, fluid balance, ultrafiltrate at visits and more
regularly PRN abnormal values. Review patients treatment flow
sheets at visits to determine effectiveness of PD treatments in
collaboration with the Multidisciplinary Team.* Assists with
discharge planning needs.* Determines dialysis adequacy (Kt/V) and
trends and reviews lab results per medical staff request and
protocol, reports abnormal/concerning labs to medical staff, and
applies changes in therapy accordingly.* Gathers data for QI and
reporting, collaborates with infection control practitioners to
trend and respond to appropriate outcome metrics.Competencies and

communication, organizational and interpersonal skills, the ability

decisions. In addition, the incumbent must be able to collaborate

Experience: 3 years experience in providing nursing care and a
minimum of 1 years experience in peritoneal dialysis therapy.
Requires knowledge of Nephrology and acute/chronic Renal disease as
well as Dialysis alternatives (Peritoneal/Hemodialysis) available.
Systems knowledge of how a team functions and serves as a resource,
preceptor, and educator about the Peritoneal Dialysis care process
to patient/family/staff. Previous experience and knowledge of
caring for patients with various disease conditions.Organizational
