Job Description:




Summary:

Registered nurse with specialized education, training and expertise in the administering injectable and infusion therapies in the home or alternative outpatient settings. Administration route for such infusion therapies are predominantly via the following routes intra-muscular, subcutaneous and intravenous (peripheral, central catheters and venous access port).



Detailed responsibilities:


  • Initiates and maintains patency intravenous lines and tubing for the safe administration of infusion therapy in the home and or outpatient setting. Ability to initiate new peripheral line and or insert new PICC or midline catheters.
  • Performs patient allergy and medication history review. Documents medication review and updates patient drug profile in the electronic charts as needed.
  • Provides training and education to patient/care giver(s) on safe parenteral administration practices and line care maintenance when applicable. Provides IV site dressing changes, practices infection control prevention measures and educates patient on signs & symptoms of infection.
  • Evaluates and interprets physician orders including laboratory testing, collection and results. Provides pertinent patient care communication to the ordering physicians and other healthcare professionals providing patient care service. Coordinates patient care activities as needed.
  • Creates individualized patient care plan to safely monitor prescribed treatment. Performs the appropriate plan of care note documentation for initial and ongoing patient care evaluations and assessments.
  • Knows how to safely use different types of parenteral delivery methods including gravity, elastomeric and ambulatory infusion pump(s).
  • Monitors patient(s) for related therapy adverse effects and reports when appropriate. Provides patient education on infusion therapy possible side effects.
  • Adheres to ethical nursing standards of practice and complies with department policies and procedures.

    • MEDICATION ADMINISTRATION: Administers medications pursuant to healthcare provider's orders and departmental policies and procedures.

    • DOCUMENTATION: Provides written/electronic record of details of patient care as appropriate to position, certification and skills.

              Required Work Experience: Two (2) years of related work experience.



              Certified Registered Nurse Infusion (CRNI) preferred.
