--;--; Summary:The Surgical Navigator is a professional member of the health care team who assumes advocacy and accountability for all surgical services patients by collaborating with the referring physicians, Surgeons, Anesthesia providers, Pre admission testing staff, OR and PACU leadership including clinical managers, Directors and managers of nursing units and Quality director and team members. --; Detailed responsibilities:--- Coordinates pre-op education along with pre admission testing staff, risk stratify patient using risk calculator tool, communicate the risk assessment with the surgeon and anesthesia providers, collaborates and communicates risk factors to all parties involved in the care of the patient.--- Coordinates post op education and home care needs prior to discharge along with PACU RN and for in patient, coordinates hand-off communication and education with inpatient nurses, tracks patient progress throughout the hospitalization and conducts post discharge phone calls to assess post-operative condition and assists with any care coordination.--- Plans and facilitates multidisciplinary meetings and communicates with the referring providers and multidisciplinary team on patient plan of care. Provides emotional support, counseling, clinical education and expert guidance to patients and families to promote their ability to understand and meaningfully participate in the healthcare process and personal decision-making.--- Provides patient and family teaching to facilitate desired patient outcomes; evaluating effects of care provided and identifying changes in patient needs.--- Develops policies and procedures, order sets, guidelines and processes to improve patient access and flow for surgical patients. Performs independently and under the direct supervision of a surgeon or an Anesthesiologist and/as appropriate other physicians.--- Continuously reviews and evaluates patient progress as outlined within the plan of care. Communicates plan of care to patient and family and solicits concerns, questions, and issues forresolution.Serves as liaison between patient and clinical staff, administration, physicians, and community/external resources to coordinate/maximize resources and identify/resolve barrierto plan of care.
